Cancellation Policy

Appointments must be canceled at least 24 hours in advance. Late cancellations or missed appointments will incur a $100 fee charged to the card on file.

Payment Policy

  • Card on file required prior to appointment

  • Payment due at time of service

Appointments are confirmed only after all required intake forms and consents are completed through the patient portal.
